Jordan Smith was Acme Corporation’s regional operations manager at the time of the incident. Her testimony primarily concerns notice of a recurring equipment fault, the company’s normal shutdown procedure, and what she learned after the incident.
The testimony supports prior internal notice of the fault. It also identifies an apparent gap in the shutdown record, but Smith did not personally observe the final inspection.
